1. Download the Word or WordPerfect version of the Intern Application, or the Word or WordPerfect version of the Sponsor Application and fill in the blanks. You may add additional pages if necessary.
2. Mail a printed copy of the completed application to:

On-line application form:

1. Complete the electronic Intern Application Form or Intern Sponsor Application Form while on-line and click the "Submit Application" button at the bottom of the form. If you want a copy of your completed form, print a copy before clicking the Submit Button.

1. Print a copy of the application form and fill in the short answer portions by hand. Use this form as a model when completing the on-line form.
2. For the essay type answers, use your word processor or text editor to compose your response, and use the cut and paste feature to paste your response into the space provided for the answer on the electronic form.
   • Be sure to skip a line between paragraphs
   • Note: The online application forms have no spell checking features.
   • Response to essay questions should be at lease three paragraphs.
3. Keep a copy of your application

1. Letters of Recommendation
   • For an intern application, two letters of recommendation are required. Each letter must be sent postal mail address given above by the person making the recommendation.
2. Transcripts
   • A copy of your transcript must be sent to the postal address given above. An unofficial copy of your most recent transcript is acceptable.
